Appointment Policy

Office hours are by appointment. Our office is open Monday through Thursday from 8:00 AM until 5:00 PM and Friday from 8:00 AM until 4:00 PM. Please note our phone lines close 30 minutes prior to close of business.

Please observe the following appointment guidelines:

24 hours notice of cancellation is required (48 hours for cosmetic, elective and surgical appointments) to avoid missed appointment fees.

New patients must arrive 15 minutes prior to their appointment to allow time for chart preparation. (Established patients with changes to personal, medical or insurance information must also arrive 15 minutes early to update their records.)

Minor children (under 18 years old) must be accompanied by a parent/guardian at the first visit or whenever there are questions regarding treatment. Children not accompanied by an adult are responsible for any co-payments due at the time of service. We will gladly take credit card information over the phone prior to the visit whenever you cannot accompany your child.

Charge card information is required to hold appointment times for cosmetic, elective and surgical appointments. If the time is missed without notice of cancellation, a missed appointment fee will be accessed. Cosmetic appointments or extended time appointments may require a missed appointment fee equal to the amount of the procedure that was scheduled.

Whenever you miss two (2) appointments without notice of cancellation, a charge card will be necessary to hold any future appointment times. If time is then missed without notice of cancellation, a $25 missed appointment fee will be accessed.

When scheduling your appointment, it is very important to let the Check-In Coordinators know all the reasons for your visit so you can be instructed accordingly.

Appointment Policy Revisions 9/9/15

Many cosmetic procedures require a deposit via charge card when your appointment is scheduled. Please contact the office for specific deposits required for cosmetic appointments.

Appointments scheduled for 30 minutes or more require a $75 deposit via charge card information over the phone at the time the appointment is scheduled or receipt of a check within three (3) business days of the appointment date.

If cancellation is received within 24 hours (one business day) prior to the scheduled appointment, the deposit will be refunded or applied as a credit on the account.
